Please Return Medicaid Consent Forms

WPS recently mailed out new consent forms to ALL families to ensure continued access to federal Medicaid funding. 
-WPS can be reimbursed for some costs associated with providing health services to students who are eligible for Medicaid programs. 
-WPS must have parental consent before seeking reimbursement. 
-Families will never be billed for school-based services and these services will not reduce the lifetime maximum available to your child through Medicaid. 

Please return this form even if you do not currently have Medicaid or if you previously gave consent. 

The form, titled "Parental Consent for Billing Public Insurance," should be completed and returned to school with your student. It can also be dropped off or mailed to the WPS central office (598 N Kent St).
